LillyDirect: Eli Lilly’s direct-to-consumer home delivery.

Pharmaceutical company Eli Lilly just announced its new telehealth care services and direct-to-consumer home delivery solutions to better enhance patient experience called Lillydirect. Lillydirect pharmacy solutions is focused on obesity, type 2 diabetes and migraines as of the announcement.

Patients can work with Eli Lilly’s recommended healthcare providers to get prescriptions, when eligible, and have third-party pharmacies’ fill prescriptions including home delivery.

As someone who has lost -74 lbs from Eli Lilly’s Mounjaro, the innovative end-to-end experience is changing  the way patients will receive care. And particularly for GLP-1 drugs like Zepbound.

Eli Lilly Telehealth

In-Person Care and Telehealth: The direct-to-consumer approach is revolutionizing the pharmaceutical industry by teaming up with telehealth providers and also in-person care to prescribe weight loss drug Zepbound and other medications.

Virtual healthcare professionals and in-person care have had friction over treatment decisions in the last few years from visits to prescription to obtaining medications, particularly with weight-loss drugs like Zepbound. Eli Lilly will be providing independent telehealth providers in its press release for company’s Lillydirect.

The convenient home delivery option will provide consistent access to patient’s medications from Lilly USA. No more shlepping out to the in-person pharmacy to only find out medication is sold out. Lilly Direct from Eli Lilly will provide solutions for patients for a variety of their everyday needs.

Eli Lilly Direct-To-Consumer

Eli Lilly will not be receiving any compensation for referring out independent doctors and healthcare professionals with its telehealth services.

From the direct-to-consumer website Thursday, “Lilly is not receiving any compensation from these 3rd-party service providers for referrals, and 3rd-party service providers are not incentivized by Lilly to promote Lilly products. The telehealth service providers that are available via LillyDirect are independent and will exercise autonomous clinical judgement in evaluating any medical conditions and resulting care decisions which may or may not include medication. Lilly evaluates potential healthcare vendors based on predefined criteria that prioritizes the quality of care, services offered, and alignment with our values. Our criteria for selecting healthcare vendors includes quality of care, service offerings, ethical, regulatory, and legal compliance, geographical accessibility, and user experience feedback.”

In addition, third-party pharmacy dispensing services will be fulfilled through accredited pharmacy service providers. The cost of medication will be a separate cost than telehealth services.

What data and information will be shared from LillyDirect to Eli Lilly? Deidentified data will be fragmented and shared from telehealth service providers.

LillyDirect says on their website, “To help us improve the Lilly consumer experience, Lilly may receive deidentified data at an aggregate level from the 3rd-party telehealth service providers represented on this site.”

Competitor Novo Nordisk

What will the new convenience of digital solutions for direct home delivery of certain drugs mean for Novo Nordisk’s Wegovy, a competitor of Zepbound and Mounjaro? About Zepbound™ (tirzepatide) injection¹
Zepbound™ (tirzepatide) injection is FDA-approved as an adjunct to a reduced-calorie diet and increased physical activity for chronic weight management in adults with obesity (BMI ≥ 30 kg/m²), or overweight (BMI ≥ 27 kg/m²) with at least one weight-related comorbid condition.

The highest dose is Zepbound 15mg. It is not a race to get to Zepbound 15mg as quickly as possible. Seek counsel to titrate up Zepbound doses. As always, Zepbound side effects may occur each time you titrate up.

Zepbound Side Effects

Side effects of the new weight-loss drug, Zepbound include:

• Nausea
• Diarrhea
• Decreased appetite
• Vomiting
• Constipation
• Indigestion
• Stomach Pain
• Gastrointestinal side effects
• Hair Loss
• Depression
• Injection site reactions

Mounjaro’s side effects included gastrointestinal problems and Zepbound will have the same gastrointestinal side effects as well.

Zepbound Cost

Zepbound will cost  $1,059.87 per month list price. Zepbound’s cost is 20% less in cost that Novo Nordisk’s Wegovy, another GLP-1.

In comparison, Novo Nordisk’s Wegovy for chronic obesity management costs: $1,349 per month’s supply.

Different commercial insurance companies are providing insurance coverage for weight loss drugs. Some insurance companies will provide coverage and a lot of insurance companies will not provide coverage for obesity treatments and new treatment options for weight reduction and substantial weight loss.

Zepbound Gastrointential Challenges

“Zepbound use may be associated with gastrointestinal adverse reactions, sometimes severe. The most commonly reported adverse events (observed in ≥ 5% of clinical trial participants) were nausea, diarrhea, vomiting, constipation, abdominal pain, dyspepsia, injection-site reactions, fatigue, hypersensitivity reactions, eructation, hair loss and gastroesophageal reflux disease.¹ 

In studies, most nausea, diarrhea and vomiting occurred when people increased their dose – but the effects generally decreased over time. In studies, gastrointestinal side effects were more common in people taking Zepbound than people taking placebo, and people taking Zepbound were more likely than those on placebo to stop treatment because of these side effects.

The label for Zepbound includes a Boxed Warning regarding thyroid C-cell tumors. Zepbound is contraindicated in patients with a personal or family history of medullary thyroid carcinoma, in patients with Multiple Endocrine Neoplasia syndrome type 2, and in patients with known serious hypersensitivity to tirzepatide or any of the excipients in Zepbound. Mounjaro vs Ozempic vs Wegovy

Both Mounjaro, Ozempic & Wegovy are once weekly injections.

Wegovy.com: Participants saw 15% weight loss or -35lbs in 1961 adults vs placebo in a 68 week study. Placebo: Participants saw 3.3% with an average weight loss of -7lbs

AIC Levels For Ozempic & Mounjaro:

Ozempic A1C levels: Lowered 1.86% on 1mg dose. 1mg is the second highest Ozempic dose. (Source)

Mounjaro A1C levels: Patients reached up to 2.3% A1C reduction with Mounjaro 15 mg. (Source) 15mg is Mounjaro’s highest dose.

From studies, Mounjaro has slightly higher data for greater weight loss than Wegovy…but only slightly.

Personally, just because I had one Mounjaro side effect at one dose didn’t mean I was going to have potential side effects for all doses.

If you are on diabetes medications, continue to monitor your blood sugar levels and discuss with healthcare professional about medical conditions.

Always seek medical help from your healthcare provider if you are having severe pain, severe stomach pain, severe abdominal pain and severe symptoms mentioned above. Those can be serious side effects and may need medical attention quickly from your health care professional. A rare side effect is thyroid cancer. You are at are at a higher risk for a genetic type of thyroid cancer if you have a familial history. Talk to your healthcare provider about symptoms of thyroid cancer and other severe side effects.

Type 2 diabetics should continue to monitor blood glucose levels as injectable medication can cause low blood sugar.
